Thank you for choosing an Advanced Physical Therapy and Wellness Center for your patient's rehabilitation needs. When your patient is in pain, the last thing that they should worry is how long they have to wait to get relief. Fortunately, out policy of scheduling patients within 24-48 hours certainly allows your patients to start their treatment right away.

We participate with most insurance companies and constantly join new ones. But, even if we find that we do not participate with an insurance plan after treatment was started, we will happily refer your patient to a participating provider and your patient will not be billed for any services provided.

For your convenience, we have included a copy of our prescription. Please print, complete and fax to us at (410) 415-0006 along with the patient's contact information. We will be happy to contact them to schedule their first appointment.
